Mucositis severity in children is most usefully captured by a scale that directly reflects oral tissue changes and how they affect a child’s ability to eat and drink. The World Health Organization mucositis scale does exactly this, ranking from no mucositis to severe mucositis with ulcers that impair swallowing and feeding. This focus on pain, mucosal appearance, and functional impact on oral intake makes it particularly practical for pediatrics, where maintaining nutrition and comfort is a priority and where simple, observable criteria are easier to apply consistently.

The other options aren’t as well suited for pediatric mucositis. A general cancer treatment toxicity scale like the NCI CTCAE can be used, but it isn’t as specifically tailored to the mouth and its functional consequences in children. The Karnofsky scale measures overall performance status, not mucosal injury, so it misses the key mucositis details. The FIFA mucositis index is not a standard, widely adopted tool for this purpose.
